Weight Loss Surgery Selector, LLC is committed to protecting the privacy of your Information. This privacy policy applies to Personal Data obtained through our Mobile Application or at www.weightlosssurgeryselector.com (“Website”) (collectively “App”). This Privacy Policy is available on our Website, where users of the App will be directed upon first use. 

“Personal Data” is defined as Information relating to an identified or identifiable person. An identifiable person is a person who can be identified, directly or indirectly, by a particular reference to an identification number or one or more factors s specific to the person. “Non-Personal Data” is defined as any Information that does not result in the identification of a specific person. “Information” may be either Personal Data or Non-Personal Data. 

The Information we collect and store when you use our App may include basic Personal Data such as name and email address. We will store this information until you ask us to delete it. You may also enter certain information necessary for our App to perform its informational service in predicting a weight loss surgery that may be an option for you, such as height, weight, health status, current health conditions, etc. (“Health Information”). We do not store, sell, or share your Health Information. 

You have the following rights related to your Personal Data: 

• to request that we disclose the Personal Data we collect and use; 
• to request that we delete this Personal Data; 
• to request that we correct any information you believe is inaccurate; 
• the right to object to us processing your Personal Data; and 
• the right to request that we transfer your Personal Data to another organization.

We do not knowingly collect or use information from users under the age of 18. If you are under
the age of 18, please discontinue use immediately and notify us at info@weightlossurgeryselector.com so that we can delete any information you have provided to us. 

The Personal Data collected by App is considered business contact information. We collect and use the Personal Data to: make marketing and sales initiatives relevant to your needs, communicate product or service information, and provide service and support or communicate significant changes to the App. We may also use Personal Data to better improve our services or to customize your experience on the App by displaying relevant informational or promotional material.  

We may use the Personal Data to send you email or other communications. All email communications, newsletters and marketing event notifications will provide you with an opportunity to opt out of receiving future emails. 

We do partner with Bariatric Surgery Providers in order to provide you with additional information should you decide to seek a professional medical opinion. Personal Data we may share with these partners is limited to your name and email address. We do not sell, trade, or rent your Personal Data to others except as provided for by this Privacy Policy. Unless we have your permission or are required by law to do so, we will only share the Personal Data you provide to us as permitted by this policy. 

We maintain reasonable physical, electronic, and procedural safeguards to protect Personal Data from loss, misuse and unauthorized access, disclosure, alteration and destruction. We maintain this information in a secure server environment that uses firewalls and other advanced technology to prevent interference or access from outside intruders. Unfortunately, even with these measures, we cannot guarantee the security of your Personal Data. By using our App, you acknowledge and agree that we make no such guarantee, and that you use our App at your own risk. 

By visiting or utilizing the App, you are accepting the policies described herein and consenting to the collection and use of Information by Weight Loss Surgery Selector, LLC. You also consent to any action we take with respect to your Information in compliance with this Privacy Policy. 

We reserve the right to change this Privacy Policy at any time by posting a new version on the Website. The new version will be effective on the date posted which will be stated at the top of the page as the new effective date. 

If we’re involved in a reorganization, merger, acquisition, or sale of our assets, your data may be transferred as part of that deal. We’ll notify you (for example, via a message to the email address associated with your account or a notice on our Website) of any such deal and outline your choices in that event.
